Providing an Integrated System of Services to People with Stroke
These standards are to be used by a stroke service in a regional, network, or multi-site setting where stroke services are coordinated among a variety of sites (acute and rehabilitation).
The standards are divided into the following sections:
- Investing in coordinated stroke services
- Integrating and coordinating stroke services
- Helping patients and families live with stroke
- Using information and information technology
- Monitoring quality and achieving positive outcomes
Accreditation Canada's Standards for Providing An Integrated System of Services to People with Stroke were developed in collaboration with the Canadian Stroke Network and are based on the current Canadian Best Practice Recommendations for Stroke Care. http://www.strokebestpractices.ca
